About Steven Smith, MD

Dr. Smith is a board-certified internist with more than 18 years of experience guiding post-acute and chronic care programs. Before joining Lifecare, he led transitional care initiatives at Emory Healthcare, designing pathways that safely moved patients from hospital to home with measurable reductions in readmissions.

At Lifecare, Dr. Smith partners closely with nurses, therapists, and social workers to customize protocols for complex diagnoses such as heart failure, COPD, diabetes, and wound care. He meets weekly with each interdisciplinary team, ensuring clinical documentation, medication regimens, and caregiver education stay aligned with family goals.
